The Program gives COVID-19 vaccines at home. Homebound vaccine program telephone number is (520) 222-0119. You can call Monday – Friday from 8:00 am - 4:00 pm.

You need to provide the following information:

  • Name

  • Date of birth

  • Address

  • Telephone number

  • What Covid 19 shot is needed, prefer Pfizer or Moderna?

  • If the person's caregiver requests Covid 19 vaccine at the time of making an appointment for the home vaccine program, the caregiver will get the vaccine too.
